Heather Cottee Privacy Policy  

Heather Cottee Physiotherapy Service of Chelmsford is the Data Controller as defined by the General Data Protection Regulation (UK Data Protection Act 20128) and this notice tells you about the information that I collect from you, why I collect it and who I might share it with.

What information do I collect from you?

When you attend the clinic for physiotherapy assessment and/or treatment I will ask you to complete a registration form with your name and contact details, including your address, email address, telephone numbers and date of birth. I will also ask you for the name of a person to contact in case of emergency and their telephone number, and the name and address of your GP and/ or your referring consultant.

I will also ask you for details of your health/ medical insurance company if you will be claiming for the cost of your physiotherapy treatment.

In addition I will ask you where you found details of the clinic (how you heard about us).

During your assessment ( and subsequent treatments) I will ask you questions about your current condition/complaint (symptoms), medical history including major illnesses and operations, general health, medication, investigations that you have or may undergo (x-rays, scans, blood tests etc..), as well as questions about your occupation, leisure activities, lifestyle and functional limitations.

Why do I collect this information?

Information on the registration form is required in case I need to contact you, your GP /consultant or your emergency contact in relation to your care.

Details of your health/ medical insurance provider are required in order for me to bill for your treatment.

I ask details about how you heard about the clinic only to help monitor the efficiency of any advertising that I use but no personal data is passed onto third parties in relation to advertising.

Information collected during your assessment and subsequent treatments is used to establish the cause of your symptoms and to help devise a treatment plan, which will be discussed with you prior to treatment commencing.

Who might I share this information with?

With your permission (by signing a consent form) I may share your information with your GP or referring consultant or with other third party referrers if I need to send letters or reports in relation to your attendance and treatment. These individuals/groups will have their own measures in place to comply with GDPR.

What do I do with your information?

The information that you have provided will be held and processed securely by Heather Cottee Physiotherapy Services of Chelmsford and will only be accessed by authorized personnel. I will only use the information that you have provided for the purposes previously stated and it will not be used for any other purpose.

With your separate consent I may occasionally contact you to inform you of new services available at the clinic or any other information that I feel may be of benefit to the improvement or management of your condition in the future by way of a newsletter.

How long do I keep hold of your information?

Legally I have to keep your medical records for a minimum of 8 years after you have finished your treatment (or in the case of children for 8 years after their 18th birthday or when 25 years old)). If storage space allows then your medical records may be kept beyond this time period, in case you return for further treatment, or a medical claim is made.

How can you obtain a copy of the information I hold about you? (Right of Access)

If you would like a copy of your medical records you must put your request in writing, including your name, address, date of birth and signature and send to Heather Cottee Physiotherapy Services of Chelmsford, Writtle Christian Centre, Church Lane, Writtle, CM1 3EN or email to heather-cottee-physio-services@supanet.com. I will usually respond within 1 month from receiving your request.


What should you do if you no longer want me to keep your personal information? (Right of Erasure)

You can ask for your information to be destroyed or deleted, as long as there is no legal reason for me to keep it. 

What should you do if your personal information is incorrect? (Right of Rectification)

You can ask for any personal information you feel is incorrect to be altered.
